Tuberculous meningoencephalitis describes a process where the tuberculous meningeal inflammatory process extends into the parenchyma of the brain resulting in encephalitis. The brain tissue underlying the exudate shows oedema, perivascular inflammatory infiltration, and a microglial reaction (border zone reaction). The clinical features include headache, neck stiffness, impaired mental state or coma, and varying combinations of limb weakness, cranial nerve deficit, and seizures. Diagnosis is established by CSF analysis which usually shows a lymphocytic pleocytosis, an elevated protein and a low glucose level. Definitive diagnosis is based on detection of bacilli in the CSF on smear or by culture. PCR may also be helpful. The presence of TB elsewhere, especially in the lungs, may also assist in making a diagnosis.
